- All other staff at your health care professional’s office
- Most-importantly-YOU! You are the most important person on your health care team.
- Patient-centered is a way of saying that you are the focus of your health care
What do you need to do as part of your PCMH Team?
Please call your Primary Care Physician’s office about care you received from other health care professionals. Call us first with questions and appointment requests before going to an Urgent Care or Emergency Room. During regular office hours, call us first with questions and appointment requests at 716-626-0030.
Let your Doctor know if you have been in the hospital. CALL OUR OFFICE AS SOON AS YOU ARE DISCHARGED FROM THE HOSPITAL, and let us know of any changes in your medications after a hospital stay or ordered by another health care provider.
- Bring all of your medications with you to each visit at our office.
- Take an active role in your own health.
- Follow the health care plan that our team has prepared for you.
- Please provide our team with feedback regarding your personalized care plan.
